The Occupational Health Program is required for all persons listed on an animal protocol or requiring access to the animal facilities. The University of Notre Dame's Institutional Animal Care and Use Committee requires documentation of participation as an item on animal protocol submission forms. The IACUC is provided confirmation of occupational health prior to the review or approval of animal use protocols through the University of Notre Dame's Wellness Center. Failure to comply with the IACUC's Occupational Health Program may delay the approval of animal protocols or personnel amendments.
The Occupational Health Program (OHP) is administered through Risk Management and Safety. All UND staff/faculty/student Occupational Health Forms are submitted online through the OnBase system. No paper copies will be accepted after November 11, 2016. Any NON-ND personnel requiring submission of Category A or Category B forms will submit a paper copy to Workforce Health Clinic using the forms below. All persons working with animals or requiring access to the animal facilities (Freimann Life Science Center or FLSC at Raclin-Carmichael Hall) must review the Occupational Health Program. All persons working at UNDERC must review the Occupational Health Program for UNDERC. After reading the above materials the powerpoint presentation, Occupational Health Program Training Deck on the Risk Management & Safety website, must be viewed and the appropriate Occupational Health Category form filled out and submitted to the University of Notre Dame's Wellness Center.
